Our programs — Adult Family Care, Shared Living, Supported Living and In-Home Support provide options for individuals with disabilities to live more independently at home and in their own community.

At UCP we offer several program options under the umbrella of Partners in Care to help support people with disabilities to remain in their own home and communities. Programs are based on each individuals level of need, the participants age and funding sources.

In each of these programs UCP also offers assistive technology and durable medical equipment evaluations and referrals to people who need it, along with educational advocacy.
